What is colloquially called "wokeism" has become a pseudo religion. The reactionary part is that it doesn't allow anybody to opt out from having a gender identity assigned to them. As long as they don't fix gender theory, so that gender-identities are optional, it has to be rejected. They are going for "Now it's my turn to impose an oppressive normativity" And that's no good.
There is scientific reality to body dysmorphia, many types exist not just the one you have talked about. There should not be ideological, political, religious or moral interference in medical treatments. As far as the Ts go there are two possible medical treatment directions for their specific type of body dysmorphia. Either the body is made to conform to the brain, or the brain is conformed to the body. This has to be decided between patients and doctors based on personal and medical criteria that give the patient the best health outcome. Any outside interference is unethical. It's likely that both treatment directions will be pursued, because statistical data suggests at least 2 different cohorts, so this phenomenon has multiple origins and will have multiple treatments as well. If you take a stance on this by suggesting either the brain or the body ought to be treated as primary, you're basically fucking over one or the other half of this group and will cause mass medical abuse like electro-shock "therapy" in the 1980s.
The reason why there is such a big fight about the definition of "women" is because parts of capital wants to change the definition so that "women" no longer includes biological reproduction. The point is to take away reproductive rights from women. This is also threatening to men because it would create legal precedent to threaten legal body self-ownership that prohibits external control over your body. If part of your body can be legally redefined to no longer be part of your legal self than capitalists might be able to compel their workers to have "work-enhancing" body modifications. The liberals imagine that their "true-inner-self" is a little pilot in their head that is steering their body, and that means they are prone to cognitive errors that could lead to a bio-tech distopia. The entire body of a person has to count as the legal self. Consider that the copyright industry would like to have DRM locks on your memories, to prevent you from remembering copyright infringing experiences. Just to be on the safe side the self should be expanded to include all the technology in your personal sphere.

But there is more to it, Ls and Gs can decline advances for sex from people higher up in the socio-economic ladder, by citing sexual preference incompatibility. Liberalism tends to get very illiberal if poor people don't offer sexual services to wealthy people.
